Case Management Services
The role of the case manager is to undertake assessment, monitoring, planning, goal setting and linking of the consumer with rehabilitation and support services.
Areas and assessments can include:
- Ergonomic Assessments
- Activities of Daily Living Assessments
- Domestic Needs Assessments
Ergonomic Assessment
Ergonomic assessments, or workstation assessments, ensure that a worker’s workstation is ergonomically designed to minimise the risk of injury and maximise productivity.
Assessment are conducted to:
- support the return to work of an injured worker by ensuring that their workstation layout minimises any discomfort as they recover from their injury and prevent any aggravation of the injury.
- for workers who work from home, to ensure their home work environment is safe and ergonomically designed.
- To provide education on correct posture, positioning of equipment and using equipment to assist in the prevention of injury .
Driving Assessments
An Occupational Therapy Driving Assessment is an evaluation by a driver trained Occupational Therapist to determine the impact of a person’s medical condition on their capacity to operate a vehicle with or without modifications.
What is involved in the assessment? Prior to referral for an Occupational Therapy Driving Assessment.
- A Doctor/Specialist needs to complete the initial medical assessment based on Roads and Maritime Services (RMS) guidelines to ensure the person is medically fit to participate in the assessment.
- If the person has a specific visual condition, a current vision test will also be required to determine if the client meets RMS vision guidelines.
Occupational Therapy Driving Assessment consists of:
- Off Road Assessment (approx.1 ½hrs)
Interview and assessment of driving history, visual, physical and cognitive skills related to driving . We are able to offer this part of the assessment at the person’s home, or in out clinic.
- On Road Assessment (approx. 1 ½ hrs)
A driving test with a driving instructor and occupational therapist to assess how the person actually performs on road.
This part of the assessment will vary subject to availability of suitable vehicle and instructor e.g. modified vehicle.
- After the assessment
Results and recommendations will be discussed with the person and a report will be provided to the RMS
